Out-of-Network Insurance Benefits

I do not currently accept insurance and am considered an out-of-network provider for benefits.

 

If your insurance coverage offers out-of-network benefits, you may be eligible for reimbursement for sessions. You will be provided with a receipt to submit to your insurance for reimbursement for services.

 

First contact your insurance provider to obtain information about your specific plan, reimbursement rate, deductible and mental health coverage.

 

Some questions to ask your insurance company:

• Do I have out-of-network benefits for mental health coverage?

• If so, what percentage is covered?

• Do I have to first meet a deductible? What is it and how much is already met?

• What is my co-pay for a session if I see an out-of-network provider?

• How do I submit a request for reimbursement?

• What is the reimbursement rate for CPT code 90834?
